Case Study: Prompt Treatment for UTI
Identifying the Need: During an EmpowerMe 360 touchpoint visit with our on-site team member, a resident mentioned symptoms of burning with urination and feeling confused. The resident felt very comfortable talking about her symptoms due to their long-standing relationship including Physical Therapy episode of care.
Community Communication: Our EmpowerMe team member informed the community’s Director of Wellness, who asked our team to inform the resident’s POA about the symptoms.
Coordination with Providers: With consent of the resident’s POA and the community’s Director of Wellness, EmpowerMe reached out to the resident’s PCP office, but the PCP and other providers were on vacation out of state.
Producing a Positive Outcome: To avoid a delay in care, our on-site team set up a virtual telehealth visit with an EmpowerMe Nurse Practitioner, who ordered urine analysis, confirmed urinary tract infection, and prescribed antibiotics – all within the same day of the symptoms being recognized. The senior living community nursing leadership expressed gratitude for our team’s prompt assistance.
