EmpowerMe is proud to be selected as a participant in the CMS GUIDE Program to provide comprehensive care to seniors with dementia. With a demonstrated history of providing impactful dementia care, EmpowerMe is part of GUIDE’s “Established Track” program and can serve enrolled seniors one year earlier than most other participants nationally.
Through EmpowerMe 360, our dementia care program delivers continuous, specialized support from our interdisciplinary team. Medicare-enrolled residents are eligible to participate in the Improved Dementia Experience (GUIDE) Model, a new approach that combines comprehensive care coordination with a strong focus on improving patient quality of life and easing caregiver responsibilities.
Care Coordination & Management
EmpowerMe’s interdisciplinary team creates a person-centered care plan involving continuous monitoring and ongoing, longitudinal care.
Care Navigation
A dedicated Care Navigator works closely with the clinical team to coordinate communication, schedule appointments, manage medications, and facilitate referrals to community services.
Caregiver Support & Education
A program for caregivers that offers training, education, clinical and non-clinical resources, and caregiver support through professional care coordinators.
Respite Care Services
Caregivers may be eligible for respite care services, offering an annual benefit of up to $2,500.
24/7 Support
Beneficiaries and caregivers can access a member of their care team or navigator anytime, anywhere.
Caregiving Training
On-demand access to comprehensive educational classes, content, and support groups on a wide array of topics related
Medication Management
The clinical team regularly reviews prescribed medication as needed, while care navigators provide tips to successfully adhere to medication regimens.
Cost Coverage
All services within the GUIDE program are fully covered for Medicare beneficiaries, removing financial barriers to essential care.
Patient Eligibility
To be eligible for GUIDE, a beneficiary must meet the following requirements:
A diagnostics of dementia
Enrolled in Medicare Parts A and B
Not currently receiving Medicare hospice benefits
Have Medicare as their primary insurance provider
Does not live in a long-term home facility (Assisted Living Facility is accepted)

If you are a traditional Medicare beneficiary and you would like to learn if you are eligible for GUIDE services, EmpowerMe clinicians can perform a comprehensive assessment and evaluation. If they determine you meet the diagnosis criteria for GUIDE they will facilitate your enrollment and ongoing participation in GUIDE. Your primary care provider or neurologist can refer you to us, however, you do not need a referral to access services. Contact your on-site EmpowerMe therapy team for guidance on enrolling.
