With our EmpowerMe 360 program, we spent over a year developing and refining a new way of delivering care in senior living communities – one in which on-site therapists become part of a truly interdisciplinary team designed to help residents access a wide range of health and wellness services. And it’s all built on a foundation of ongoing relationships between our therapy teams and the seniors they serve.

Combining Therapy with Care Coordination

EmpowerMe 360 is an innovative new program designed to bring all the advantages of a health system right to our residents’ fingertips. It goes beyond the role traditionally played by therapy providers, expanding how we work with our communities’ residents to create extended, or longitudinal, care relationships in which we provide truly comprehensive health and wellness services to meet all our residents’ needs. This includes:

Conducting regular one-on-one health and wellness visits to help seniors work toward their goals
Helping develop customized wellness plans for each resident
Proactively communicating with residents’ physicians, specialists, caregivers, and family members
Facilitating telehealth visits with EmpowerMe physicians and specialists that provide expertise in over 125 specialties

EmpowerMe 360 allows us to be an extra set of eyes and hands to help all our residents. It creates much more of an interdisciplinary approach within the community, not only among the EmpowerMe team members, but with the community staff, the resident’s providers, the resident’s family members, and the residents themselves.

Deeper Relationships Mean Better Care

EmpowerMe Wellness has always offered some of the lowest productivity expectations in the industry. We do this to encourage our therapists to take additional time with residents to get to know them and more fully understand their care needs. With EmpowerMe 360, that additional time for relationship building becomes built into therapy teams’ weekly routines, and counts toward their productivity goals.

The results of introducing care coordination into EmpowerMe’s service model has been impressive. Already, EmpowerMe 360 has shown to reduce falls by 30% compared to the national average. Residents served in EmpowerMe 360 communities also experience 27% fewer hospitalizations.

EmpowerMe 360 enables me to spend one-on-one time with my residents to check in on them and ask them how they’re feeling. Sometimes I can pick up on things that they’re not able to voice to me. Sometimes they may say they’re okay, but I can see they’re not really okay. And that’s made a huge difference in the care I provide.
