A common goal, an individualized approach. Our approach to therapy and wellness services is a simple and consistent one. We begin by listening to the seniors we will work with. In addition to learning their individual goals, needs and preferences, we get to know them as human beings. From there, we create a unique therapeutic framework for each, built around (and inspired by) their unique life. Through deep knowledge of each patient’s daily realities, our therapists are able to define tangible life goals, and create a custom path and plan to get them there.

It is the goal of every therapist on our team, every service in our offerings, and every moment spent working with seniors: to restore and preserve their independence. That is our constant, unwavering aim and passion.

Physical Therapy

For Balance, Fall Reduction, Strength Exercises, Neck & Low Back Pain.

No matter your age or ability level, one truth holds: the better your body works, the better you feel every day. That’s why our physical therapy services focus on getting seniors moving again.

Our approach is to listen to what each resident’s goals are, and frame their therapy program around those real-life goals. Our personalized, one-on-one therapy programs may be focused on a goal of lifting a grandchild, walking a dog or better independence in the kitchen or bathroom.

Regardless of the cause of limited mobility – injuries to the musculoskeletal system, post-operative orthopedic rehabilitation needs, disorders, aging or more, our therapy staff is trained to help seniors restore use of the body systems that have been compromised.

Constant and consistent documentation is also an important component of our therapy. It’s our strong belief that charting progress requires far more than a checklist. Our therapists are trained to actively listen to the evolving needs and preferences of each individual patient. We can reposition a patient’s course of treatment based on the nuances of their progress, keeping the community informed of every triumph and challenge relative to their goals.

Resident Goal: “Help me keep my dog with me”

An EmpowerMe Wellness resident in Assisted Living was encountering mounting challenges caring for her beloved dog Poncho. Our customized therapy program focused on gait, balance and strength training with goals relating to:

  • Holding a leash while remaining balanced
  • Bending over to feed and water Poncho
  • Walking Poncho for increased distance and frequency

The therapeutic regimen has worked and the resident has happily continued to be able to provide for Poncho. In fact, therapy has become one of her favorite activities, knowing her goal of greater independence and staying with her dog has been realized.

Our Physical Therapy offerings include:

  • Daily exercises for endurance and strengthening
  • Treatments for swelling, joint pain, and limited range of motion
  • Balance training and fall reduction

Occupational Therapy

For Self-Care, Posture, Wrist & Hand Pain, Mobility Issues.

Occupational therapy focuses on resident goals involving the activities of daily living – restoring abilities surrounding writing, eating, dressing or bathing, and any activities requiring fine motor skills such as buttoning a shirt or tying shoes. These simple skills can become especially challenging in the wake of an injury or illness.

Restoring physical capabilities and improving dexterity are not the only components of a successful treatment plan helping get seniors “back to normal.” Occupational therapy also helps in the emotional and social adjustments following an illness or injury to help seniors regain independence, self-reliance, and confidence.

The Halloween holiday offered the EmpowerMe therapists the opportunity to gather senior residents for an engaging contest as well as a group therapy session. Our contest included a therapy program focused on balance, hand strength, dexterity and fine motor skills relating to:

  • Lifting a pumpkin onto a table
  • Handling a spoon to scoop out the pumpkin
  • Dexterity involved in cutting out pumpkin features

A fun social event provided measurable results for therapists to observe and record for each resident participant, and kept therapy fresh and inviting for everyone involved.

Our Occupational Therapy offerings include:

  • Training on functional skills such as dressing, bathing and using the restroom
  • Support to improve a senior’s ability to transfer in and out of the tub, shower, bed and car

Speech Therapy

For Communication, Swallowing, Memory Concerns.

The EmpowerMe speech therapy program addresses all aspects of speech and oral disorder, assisting seniors and their families with solutions to remedy challenges related to language, cognition, speaking, swallowing and eating. We can help in differentiating between normal aging and disordered communication or swallowing function.

Being able to communicate is vital for seniors, in describing a particular need or where something hurts, or in avoiding a potential emergency. Our speech therapy program treats language, cognitive, speech and feeding difficulties resulting from accidents, strokes or neurological conditions. Speech therapy also applies to memory loss. We teach compensatory strategies for deficits as they relate to communicating, and focus upon formerly intuitive behaviors like swallowing.

Language and Cognition Go Hand-In-Hand

Standardized cognition stimulation assessments help our therapists understand where the resident is within the disease process and then develop a custom solution. With a philosophy of “have a job, have a purpose,” strategies are created with specific goals in mind:

  • A resident may want to continue to pay their own bills
  • Crafting or playing bingo may be an ongoing passion
  • Communication with family via technology may become a priority

For example, using a smartphone as a tool, and a goal of keeping in touch with a grandchild, a therapy can be created involving following a sequence of steps, repeating those steps, speaking on video chat or on the phone, remembering a scheduled call time and the like. We always view EmpowerMe as part of the larger community team, and design the therapies so caregivers can follow through as well.

Our Speech Therapy offerings include:

  • Communication: Addressing expressive and receptive skills is key to quality of life. Here, the individual attention of our therapists is especially vital, as no two people communicate in precisely the same way.
  • Swallowing: Improving swallowing skills through muscular strengthening. While many wouldn’t normally associate this with “speech,” any challenges relating to the mouth and throat can be assessed and treated by our speech therapists.
  • Memory: Strategies for impacting short- and long-term memory. This is dependent upon an informed evaluation of where the patient is in the dementia process, as well as our personal knowledge of the patient. Our therapists’ refined soft skills and personal connections with patients make the process more productive and pleasant.

Dementia Therapy

Our Innovative Dementia Care Model: THE FOCUSED MIND

Identifying functional capabilities with a focus on remaining abilities.

Providing care for residents living with dementia is an integral part of the person-centered environment, which is necessary to support a high quality of life with meaningful relationships and engagement.

The Focused Mind Program was designed to support residents through implementation of individualized approaches to care. These approaches are directed toward understanding, preventing, relieving, and accommodating a resident’s loss of abilities. The goal is to attain or maintain his or her highest practicable level of well-being.

With the Focused Mind Program, all residents with cognitive impairment are evaluated by our therapists using Allen Cognitive Testing and other standardized assessments to determine the individual stage in the disease process.

Allen Cognitive Testing

Allen Cognitive Testing is unique compared to other cognitive testing due to the focus on FUNCTION. The Allen Cognitive Theory, or Cognitive Disabilities Model, was developed by Claudia Allen, an Occupational Therapist. The model enables us to identify a person’s functional cognition at every stage of dementia, so that we can focus on remaining abilities rather than on the disease process or on things the person can no longer do. Unlike other cognitive models or assessments, Allen Testing can be done even when the person has lost the ability to communicate or write.

What is unique about The Focused Mind is what we DO with the information gathered from the assessment.  Rather than gathering information for a baseline, we take that a step further and have a set protocol for developing and implementing education and individualized maintenance programming for the resident.

Based on identified retained functions, our therapists educate family and caregivers on personalized techniques and strategies to maximize independence, promote effective communication, encourage cognitive stimulation, and improve quality of life.
