Getting Started With Care
Before services begin, there are a few important steps that help us confirm your information, understand your insurance benefits, and make sure the appropriate forms are completed.
Understanding Your Consent Forms
Before receiving care, you or your authorized representative will be asked to review and sign required consent forms.
These forms confirm that you agree to receive services, allow our team to coordinate your care, and authorize us to bill your insurance when applicable.
In most cases, required consent forms should be completed before your first visit. You may occasionally be asked to complete updated forms if your care changes or additional information is needed.
If Someone Else Helps Manage Your Care
If a power of attorney (POA) or other authorized representative helps make healthcare decisions on your behalf, they may complete required consent forms electronically.
Checking the Status of Your Forms
You can review the status of your documents through the Alinea patient portal. Any forms that still need to be completed will appear on your dashboard.
Completed and signed forms can also be accessed through the portal whenever you need them.
Understanding Your Expected Costs
Before care begins, you may receive a Good Faith Estimate or Benefit Summary outlining your expected out-of-pocket costs based on the information available at the time of intake.
You can access these documents through your Alinea patient portal.
Using the Alinea Patient Portal
Alinea is a secure online portal that helps you manage important information related to your EmpowerMe care.
Through Alinea, you can review and sign consent forms, access information about your insurance benefits, submit insurance changes, and securely communicate with your EmpowerMe team.
How Will I Receive Access to Alinea?
When it’s time to review or complete documents, you may receive a secure link by email or text message.
Look for an email from [email protected] or a text from (414) 988-3062 identifying EmpowerMe Wellness and your community.
The first time you access the portal, you’ll be prompted to create a password. From there, you can review and complete outstanding documents from your dashboard.

Understanding Your Insurance
Your insurance coverage can affect what you pay and when you receive a bill. Use the information below to understand your coverage, update your insurance information, review your Explanation of Benefits (EOB), and know what to expect if a claim is denied.
Does EmpowerMe Accept My Insurance?
Insurance coverage can vary based on your individual plan, location, and the services you receive.
The best way to understand whether your insurance covers EmpowerMe services and what you may be responsible for paying is to speak with the EmpowerMe Clinic Director at your community. They can help connect you with the appropriate team to review your specific coverage.
What If My Insurance Changes?
Please let us know as soon as possible if your insurance coverage changes so we can update your information and help prevent disruptions or billing delays.
You can provide your updated insurance information by speaking with your EmpowerMe Clinic Director, submitting an Insurance Benefit Change through the Alinea patient portal, or contacting our Patient Advocate Team.
Understanding Your Explanation of Benefits (EOB)
After your insurance company processes a claim, you may receive an Explanation of Benefits, commonly called an EOB.
An EOB is not a bill.
It explains how your insurance company processed the claim, including the amount billed, the amount your insurance allowed, what your insurance paid, and the amount that may be your responsibility.
What If My Insurance Denies a Claim?
If your insurance company denies a claim, you may not need to take any immediate action.
Our team reviews insurance denials and works with the insurance company to understand the reason for the denial and determine whether additional information or corrections are needed.
If we need information or action from you, we will reach out.
If you have questions about a specific claim, contact our Patient Advocate Team.
How Your Insurance Processes a Claim
Your EOB shows how the amount billed is processed by your insurance and how your portion of the cost is determined.
See What an EOB Looks Like
Every insurance company formats its EOB differently, but most include similar information about your claim, the services provided, what your insurance paid, and the amount that may be your responsibility. Use this sample to see where you might find that information on your own EOB.
Sample shown for educational purposes only. Your EOB may look different depending on your insurance plan.

Understanding Your Bill
Your bill may arrive several weeks after you receive care, and the amount can vary from one visit to another. The information below explains what to expect, why charges may differ, and why you may receive more than one EmpowerMe bill or account number.
When Should I Expect My Bill?
Your first statement may arrive approximately two to four months after services begin, depending on your insurance coverage and how long it takes your insurance company to process the claim.
EmpowerMe waits for applicable insurance claims to be processed before generating your statement so it can reflect the most accurate information available about what you may owe.
After billing begins, statements are generally sent every four to five weeks.
Why Can the Amount Be Different From One Visit to Another?
The amount billed may vary from one visit to another based on the type of services you receive, the amount of time spent providing those services, and the specific billing codes associated with your care.
If you have questions about charges for a specific date of service, our Patient Advocate Team can help review your billing with you.
Why Did I Receive More Than One Bill or Account Number From EmpowerMe?
EmpowerMe offers several healthcare services, including EmpowerMe Rehabilitation, EmpowerMe Medical Group, and EverSpring Pharmacy.
Because these services are billed separately and may use different billing systems, you may receive separate statements and account numbers depending on the services you receive.
If you have questions about a statement or aren’t sure who to contact, our Patient Advocate Team can help connect you with the appropriate team.
From Care to Statement
Before your statement is sent, EmpowerMe submits the claim to your insurance so it can be processed and your responsibility can be determined.

Common Billing Situations
Some billing situations may require a few additional steps. Find guidance below for common questions or situations that may come up.
My Insurance Company Sent Me a Check. What Should I Do?
Sometimes an insurance company may send payment for EmpowerMe services directly to you instead of sending it to EmpowerMe.
If this happens, you’ll need to provide EmpowerMe with information from the accompanying Explanation of Benefits and send the insurance payment to EmpowerMe so it can be applied correctly to your account.
Click the button below for the steps.
My Insurance Claim Was Denied
In many cases, you do not need to take immediate action when an insurance claim is denied.
Our team will review the claim and work with your insurance company to determine what additional information or corrections may be needed.
